Position Summary:

The PACE Clinic Nurse Manager is responsible for managing the daily clinical operations of the PACE Fort Myers medical clinic, ensuring that all clinical staff and services adhere to PACE regulations, policies, and standards established by Hope PACE. This role works collaboratively with various PACE department managers, including Therapy Services, MSW, Homecare, and Transportation/Facilities, to deliver high-quality care and improve performance across the program.

Key Responsibilities:

  • Leadership & Management:
    • Supervise clinical staff, including hiring, training, and performance evaluations, to ensure they meet PACE care standards.
    • Oversee daily clinical operations and work collaboratively with other department managers to solve operational issues.
    • Participate in the PACE administrative on-call rotation, providing leadership and decision-making support after hours.
  • Patient Care:
    • Monitor consistent care delivery in the clinic and participants' homes, ensuring that established care protocols are followed.
    • Collaborate with the Medical Director, Director of Nursing, physicians, and the interdisciplinary team to develop and maintain nursing practice standards.
    • Interface with members, families, and contracted providers to resolve care-related concerns and improve service delivery.
  • Quality Assurance & Compliance:
    • Ensure compliance with infection control, safety protocols, and employee health policies.
    • Collaborate with Quality Assurance and Performance Improvement (QAPI) staff to track, report, and address clinical incidents, including member falls and complaints.
    • Conduct regular chart reviews and incident reporting to maintain compliance with PACE and regulatory standards.
  • Operational Management:
    • Authorize procurement and oversee inventory management of medical supplies, medications, and equipment.
    • Monitor and maintain the clinic budget in collaboration with the PACE Director of Nursing, ensuring fiscal responsibility and performance monitoring.
    • Coordinate with the Homecare Manager to address homecare needs for participants, ensuring seamless care delivery across settings.
  • Collaboration & Communication:
    • Work closely with other PACE program managers to maintain consistent, high-quality service delivery, including managing participant appointments, transportation needs, and home care services.
    • Review nursing feedback, incident reports, and care documentation, communicating with the interdisciplinary team to ensure consistent care and follow-up.
  • Professional Development & Education:
    • Participate in assigned agency committees and external associations to enhance professional knowledge and contribute to organizational improvement.
    • Provide mentorship and guidance to nursing staff, ensuring ongoing education and professional development in accordance with PACE requirements.
  • Competencies:
    • Leadership: Demonstrates effective leadership, problem-solving, and conflict resolution skills.
    • Communication: Excellent verbal and written communication skills, with the ability to collaborate effectively across teams.
    • Time Management: Demonstrates strong organizational and time management skills, ensuring efficient clinic operations.
    • Technical Proficiency: Proficient in Excel, Microsoft Office, Outlook, and electronic medical records (EMR).
    • Team-Oriented: Committed to working in a team-based care model and upholding high ethical standards.

Qualifications:

  • Education: Registered Nurse (RN) required. Bachelor’s degree in Nursing (BSN) preferred.
  • Experience: Minimum of two years of leadership experience in a healthcare setting, with knowledge of PACE programs or geriatric care preferred.
  • Licensure: Active RN license in the state of Florida. Valid driver’s license and auto insurance required.
  • Certifications: CPR/BCLS certification required. Mandatory Reporter certification maintained.

Work Environment:
The PACE Clinic Nurse Manager works in a clinical environment with occasional travel to participants’ homes. This role involves physical tasks such as standing for extended periods and assisting with patient care as needed.
