Job Summary

The department of Internal Medicine -Infectious Disease is seeking a Program Coordinator I to serve as a member of the multidisciplinary care team in order to provide services to patients.

Provide information and assistance with linkage to medical case management and other social services as needed. Screen and determine eligibility of clients for emergency financial assistance according to program protocol.

Maintain necessary documents regarding proof of hardship and maintain system to track service limitations. Obtain necessary documentation for payment (invoice, W9, vendor application form). Collaborate and communicate with the leadership team to ensure completion of referral and timeliness of payment.

Triage walk-ins for Medical Case Manager program and connect to services as appropriate. Ensure appropriate documentation is available in client files including documentation of hardship, service limitations, and completion of appropriate program notes and documents.

Communicates verbally and in writing regularly in the patient record, concerning patient care activities and inquiries.



Skills / Knowledge / Abilities

MS Office Suite (Word, Access, Excel, Outlook), CAREWare.

Preferred Education/Experience

Prefer Human/Social Services education background. Prefer experience working directly with and providing services to clients.
