Job Profile Summary
Role:
The Supervisor, Continuous Care is responsible for direct oversight of all aspects of patient care provided by the Continuous Care staff members including personnel supervision. Provides and manages direct care to patients.

Qualifications:

  • Current Florida license as RN.
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross. 
  • Minimum of three (3) years’ nursing experience
  • One (1) year or more of supervisory/team lead experience preferred
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
  • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
  • Excellent critical decision making and problem-solving skills
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to effectively communicate in English
  • Flexible schedule to meet business needs and extended hours oversight
  • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area the Company serves
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Job Responsibilities:

  • Directly supervises 24X7 Continuous Care team covering multiple locations to ensure that the patient care delivery is efficient, effective, and achieves desired patient clinical outcomes.  Ensures staff provides  appropriate nursing intervention and follow-up. 
  • Coordinates employees and patient care to ensure that the needs of the patient and family are met using best standards of practice and in an efficient manner.
  • Recommends alternate approaches to patient care as indicated while ensuring that policies and procedures are followed.
  • Provides education to team members regarding hospice/palliative philosophy, symptom management, case management, and organization
  • Provides performance feedback and monitors accountability of clinical staff through documentation audits, reports, quality monitoring and supervisory visits.
  • Enhances the clinical practice of team members by performing as a role model, mentor, instructor, and resource person.  Primary clinical resource for department staff.
  • Collaborates with other departments (i.e., Medical Services, Psychosocial staff, Pharmacy, Operational Teams. and Phone Triage to ensure overall patient care needs are addressed. 
  • Demonstrates respect and understanding for all clinical disciplines required for quality patient care and mentor staff in this process.
  • Provides direct nursing care to select patients, as deemed appropriate, to maintain own clinical competence assess particular patient requirement, provide modeling to staff and to meet business needs.
  • Communicates in a clear and accurate manner  Provides constructive feedback and resolves conflict directly.  Assists others to improve communication skills.  Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others.
  • Ensures timely attention to customer concerns and complaints. 
  • Provides excellent customer service to both external, and internal customers.
  • Assists in reviewing patient care documentation and providing clinical supervisory visits and clinical competencies to identify strengths and areas of development for the clinical staff.
  • Availability before, during, and after emergencies impacting Chapters affiliates, i.e., hurricane
  • Performs other duties as assigned.
