Our employees make all the difference in our success!Role: The Physical Therapist position is an integral member of the dedicated inter-disciplinary team that provides important short term / long term care of our patients. They provide a high level of care for our patients in the client homes and in facilities. This is a Full Time position working days Monday – Friday. • Key Responsibilities o Provide &amp; direct physical therapy services o Initiate physical therapy program &amp; instruct other personnel &amp;/or family/caregivers in certain phases of the physical therapy with which they may work with the patient o Evaluates &amp; assesses patients o Documents initial evaluations, progress notes, daily records in participant Medical Records, including Plan of Care o Collaborate with other team personnel to achieve well-rounded care • Knowledge &amp; Skills o Excellent communication; via phone, in person and email o Attention to detail &amp; ability to handle confidential information following HIPPA guidelines • Required qualifications o Bachelor’s degree required &amp; Master’s program in Physical Therapy preferred, accredited organization o Minimum 1 year experience working with frail / elderly population o 2 years experience as a Physical Therapist o Community / home health experience preferred o Valid Florida Drivers’ License with good record o Automobile Insurance o BLS Certification from the American Heart Association or Red Cross o Clear level 2 background checkQualifications: • Graduated after successful completion of a physical therapist education program approved by one of the following: o The Commission on Accreditation in Physical Therapy Education (CAPTE) o Successor organizations of CAPTE o An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists • Licensed in the state of Florida- CPR certification is required • Minimum of two (2) years’ experience working in an acute care setting • Mobile Driver - Valid driver’s license and automobile insurance per Company policy • Able to travel and have reliable transportation to meet visit schedule • Ability to use equipment with visual and auditory mechanisms • Ability to effectively communicate in English (written and verbal) • Flexible schedule to meet the needs of patients • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area Company serves • Ability to manage stress • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation Competencies: • Satisfactorily complete competency requirements for this position. Job Responsibilities: • Provides physical therapy evaluation, treatment, and interventions to program patients. • Conducts initial assessments and reassessments, upon referral from team, of patient’s condition and functional status and establishes measurable goals and a treatment plan. • Provides training, guidance, and supervision to program staff and patients/family members regarding therapy. • Responsible for providing and recommending therapeutic exercises. • Participates actively in the Interdisciplinary Group (IDG) by providing clinical expertise in team planning, problem solving, and processes. • Fulfills all administrative requirements including, but not limited to, collaboration with Occupational Therapist, attendance at team and staff meetings, and recording and tracking of physical therapy durable medical equipment (DME). • Develops treatment program, when indicated, sets goals and provides treatment. • Records and signs treatments in patient’s medical record; records, signs, and dates clinical documentation. • Reviews assessments and discharges patients, when appropriate. • Performs other job duties as assigned.This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
